Voluntary disenrollment

You may end your participation in Fallon Health Weinberg-PACE at any time. To disenroll, contact the Fallon Health Weinberg-PACE site director, social worker or a member of your Interdisciplinary Team.

Enrollment in a PACE continues regardless of changes in health status unless the participant voluntarily disenrolls or a PACE organization involuntarily disenrolls the participant for strictly defined reasons listed below. A PACE participant may voluntarily disenroll from the program without cause at any point. Voluntary disenrollments are effective the last day of the month after the request for the disenrollment is received. The participant will remain enrolled in Fallon Health Weinberg-PACE and be fully covered until the last day of the month. The disenrollment date will be coordinated between Medicare and Medicaid (as appropriate). No disenrollment will become effective until the participant is appropriately reinstated into other Medicare and Medicaid programs (as appropriate) and alternative services are arranged.

The PACE organization may only involuntarily disenroll a participant for any of the following reasons:

  • Failure to pay
  • Disruptive or threatening behavior, such as behavior that jeopardizes the participant’s own health or safety, or the safety of others
  • Consistent refusal to comply with an individualized care plan or the terms of the PACE program Member Handbook/Enrollment Agreement
  • Relocation outside of the Fallon Health Weinberg-PACE service area

All services must be provided by or authorized by the PACE Interdisciplinary Team (except emergency services). PACE participants may be held liable for costs for unauthorized or out-of-PACE program agreement services.
