Fallon Health Weinberg-PACE is a Program of All-inclusive Care for the Elderly (PACE). The PACE model of care offers comprehensive medical; prescription drug coverage; speech, physical and occupational therapy; and other social services to adults 55 and older.

Fallon Health Weinberg-MLTC

The Fallon Health Weinberg-MLTC plan has closed. If you are a former member and need a copy of your medical records, please call us at
1-866-882-8185 or 1-716-250-3100 (TTY 711), 8 a.m.–5 p.m., Monday–Friday.

All services must be provided by or authorized by the PACE Interdisciplinary Team (except emergency services). PACE participants may be held liable for costs for unauthorized or out-of-PACE program agreement services.
