Fallon Health offers tours during National PACE Month

Worcester, Mass., September 1, 2024—In celebration of National PACE Month this September, Fallon Health invites families to explore why Programs of All-Inclusive Care for the Elderly (PACE) are an excellent choice for helping older adults maintain independence in their own homes. This year’s theme, “Simplify Your Life: At Your PACE,” highlights the ease and benefits of this comprehensive care model.

Backed by nearly 30 years’ experience, the nationally recognized not-for-profit health care services organization has grown its Massachusetts PACE program, Summit ElderCare®, to today include 5 PACE Centers serving 10 counties across the Commonwealth. With a focus on expansion, Fallon Health is actively working to increase its presence and services through Massachusetts. It is the largest PACE program in New England and among the largest in the country. It also operates a program in Western New York called Fallon Health Weinberg-PACE which serves eligible individuals in Erie and Niagara counties.

PACE is a program and model of care that allows people to live independently in their homes and communities rather than in nursing homes. With a focus on quality of life and individualized, patient-centered care, the program offers dedicated caregivers, transportation, full medical care, and opportunities for social interaction, ensuring participants can live fulfilling lives free from isolation—with all their care needs met.
