Benefits and services
Our complete and coordinated services help older adults who need a nursing-home level of care, but wish to stay living at home and in their community.
With Fallon Health Weinberg-PACE, you may receive:
- Comprehensive medical and specialty care
- Services at an adult day program
- Prescription drug coverage
- Medical transportation
- A Care Team of geriatric experts (also known as an Interdisciplinary Care Team)
- Home care
- Caregiver support
Medical transportation
Transportation to and from the PACE Center or off-site medical appointments may be available. Our transportation providers work closely with you and your Care Team to address your individual needs.
Home care
Fallon Health Weinberg-PACE may provide in-home help with your daily activities, based on your needs.*
Home care services may include:
- Help with bathing and dressing
- Medication management
- Emergency response systems
- Light housekeeping
- Help with meal preparation
- Other resources as needed
* Your Care Team will work to develop an individualized care plan based on your needs.
Getting care at the PACE Center
Once you enroll in the program, you’ll work with a Care Team to identify and evaluate your needs. We'll create a comprehensive plan of care that's personalized to help you remain safely at home and in the community. All of your medical and specialty care is coordinated by your Care Team.
Your Care Team and individualized plan of care
Your Care Team includes care professionals with geriatric experience. The team may include a primary physician, nurse practitioners, nurses, specialists, social workers, rehabilitation staff, activities coordinators, health aides, home care specialists, a nutritionist, and others.
PACE Center
You may receive most of your care (including medical visits, routine lab work, medical treatments and services like physical therapy) on site.
It's a place where participants can safely socialize, enjoy meals and snacks, and receive care from our staff.
In the PACE Center, we offer:
- Medical care through the on-site clinic
- Meals and snacks
- Rehabilitation services like physical therapy, occupational therapy, and speech therapy
- Social interaction and engagement
- A variety of stimulating activities developed to be restorative, rehabilitative, meaningful, and interactive

Caregiver support
Being a caregiver is an important role. We partner with you and provide the needed support to keep your loved one safe and as independent as possible.
At Fallon Health Weinberg-PACE, we support you through:
- Individualized consultation on your loved one's care plan
- A dedicated Care Team familiar with your loved one's needs
- 24-hour phone access to a registered nurse
All services must be provided by or authorized by the PACE Interdisciplinary Team (except emergency services). PACE participants may be held liable for costs for unauthorized or out-of-PACE program agreement services.
