It's easy to register for Provider Tools. Complete just one registration form to access all of them!
Step one:
Download the Provider Tools Registration Enrollment Form (pdf), which includes the provider enrollment form and Billing Agency Authorization form.
Step two:
Complete the requested information.
Step three:

Upon receipt, Fallon Health Weinberg will verify your provider status and assign you a username and password. We will contact you with your personal information.
