Weinberg Campus partners with Massachusetts health care company to expand services

Joint venture created with Fallon Health to expand services provided by Total Aging in Place Program in Western New York State

Amherst, NY, October 27, 2014 - Weinberg Campus, our region’s largest aging health campus, today announced that it has formed a joint venture with Fallon Health of Massachusetts to expand the services currently provided through its Total Aging in Place Program (TAIPP) in Erie County in Western New York state. Effective immediately, TAIPP has been renamed Fallon Health Weinberg.

Recognizing the needs of an increasing number of elders in the region, Weinberg Campus is extending its services further into the community. Weinberg reached out to a nationally recognized not-for-profit health care services organization with extensive experience in caring for adults over 55 with complex care needs.

The joint venture, Fallon Health Weinberg, will bring together each organization’s capabilities and expertise in delivering high-quality, person-centered, cost-effective health care programs to individuals who require supportive services due to long-term health needs. There will be a number of programs offered under the Fallon Health Weinberg umbrella.

The first is TAIPP which was established in 2003. TAIPP is a Managed Long Term Care plan (MLTC) and a provider of community-based long-term care services. The program is designed to help participants to continue to live safely in their own homes and communities. The program provides and coordinates needed health care and supportive services to promote independence.

In support of the planned expansion of services, Fallon Health Weinberg has applied to expand this MLTC plan to serve eligible individuals 21 years and over in both Erie and Niagara counties.

In addition, it also has applied to become a provider of a Program of All-inclusive Care for the Elderly (PACE), and will soon begin marketing a Medicare Advantage Special Needs Plan (HMO SNP) for dual-eligibles in October for a January 1, 2015, enrollment date. PACE is a Medicare and Medicaid program that helps people age 55 and older meet their health care needs in the community instead of going to a nursing home or other care facility. Fallon Health Weinberg’s HMO SNP is a Medicare Advantage plan for individuals 21 years and older who are eligible for both Medicare and Medicaid.
